I/we acknowledge that I/we understand the intent thereof, and I/we hereby agree and absolve and hold harmless the Stollery Children's Hospital Foundation, Alberta Children's Hospital Foundation, corporate sponsors, cooperating organizations and any other parties connected with this event in any way, singly or collectively from and against any blame and liability for any injury, illness, misadventures, harm, loss, inconvenience or damage thereby suffered or sustained as a result of participation in the 2023 Life Cycle or any activity associated therewith. I/we hereby consent to and permit emergency treatment in the event of injury or illness. I/we also give full permission to use my/our name and/or photo in connection with this event.

I/we give my/our expressed consent to receive electronic news and marketing materials associated with Life Cycle 2023 and subsequent Life Cycle events.
